This position is directly responsible for the implementation and coordination of community development and support work in the Halton Region.

The objectives of this program and position are to:

  • To reduce the isolation and social stigma experienced by people living with HIV/AIDS in the Halton Region
  • To increase the coordination of HIV/AIDS care and support services within the Halton Region
  • To increase the points of access and availability of services to people living with and or affected by HIV/AIDS living in the Halton Region
  • To increase HIV/AIDS knowledge and information materials available to service providers in the Halton Region.
  • Working through a community development framework in partnership with key partners in Halton and provincial networks.


  • Post-secondary degree/diploma in social services, community development, health promotion or a related field.
  • Demonstrated experience working in community based settings.
  • In-depth knowledge of issues facing people living with HIV/AIDS in Halton Region;
  • In-depth knowledge of social service, healthcare and recreational providers in the Halton Region;
  • Previous experience conducting presentations to social service agencies, healthcare providers and community services etc.;
  • Excellent organizational skills and attention to detail;
  • Flexible to challenges as they arise and the ability to remain calm under pressure;
  • Ability to handle confidential/sensitive information with discretion and maturity;
  • Demonstrated ability to communicate effectively in conflict and other situations that may require tact, diplomacy and discretion;
  • Strong communication skills in English, both written and oral;
  • Demonstrated proficiency with computer systems, (i.e., PowerPoint, Word, Excel, Outlook, Web-based Client Management and Reporting Systems);
  • Must have a valid G Class driver’s license, clean driving record with access to and use of an automobile is required.


  • Intake, assessment and support services to individuals and families who are living with/affected by HIV/AIDS within the Halton Region;
  • Case management and referral services with key partner agencies including, but not limited to, the SIS Clinic, healthcare providers in Halton, OW/ODSP programs, housing providers, etc.
  • Developing peer engaged support programs;
  • Provision of HIV/AIDS training for local social service agencies and healthcare providers, working with priority population programs within The AIDS Network for people who may be at increased risk of acquiring HIV or staying engaged in HIV treatment;
  • Working in partnership with The AIDS Network’s Halton-based harm reduction team to support people living with and at risk of acquiring HIV who use substances.
  • Development of partnership and referral agreements between social services, health care and The AIDS Network;
  • Development of locally based and relevant promotional materials related to HIV/AIDS;
  • Other areas as approved by management.

This position is based out of Oakville and provides services throughout Halton Region.
